Mental health needs

LD Care supports people with multiple diagnoses of learning disabilities and mental health needs.

We liaise with local healthcare professionals to ensure that consistent and appropriate input is available. We also have excellent working relationships with community mental health and learning disability teams.

LD Care ensures that the support we provide is flexible, based on individual needs and in line with the Care Programme Approach (CPA). Person centred planning enables us to undertake detailed care and relapse prevention planning whilst measuring progress by working towards a set of realistic outcomes. These outcomes are identified by everyone we support, which is a fundamental part of gaining greater independence.

We appropriately safeguard people with complex multiple diagnoses whilst supporting them to exercise full choice and control using positive means of managing risk including Pro-active Risk Management. We use clear, multi-agency risk assessments and strategies together with adequate support levels of supervision to facilitate inclusion.

In addition to our comprehensive mandatory training, our teams undertake specialist training focusing on mental health and learning disabilities needs.
