The LD Care Approach

Our behaviour management approach at LD Care reinforces our caring ethos by focusing on proactive engagement rather than reactive responses. We prioritise interaction and keeping individuals engaged as a way to prevent behaviours from escalating.

Central to this approach is developing trust and strong relationships with our residents, ensuring they feel safe, secure, and understood. By knowing and implementing their routines, preferences, and likes as detailed in their support plans, we provide consistent, compassionate support, even when behaviours arise. This method not only helps to prevent challenging behaviours from occurring but also strengthens the bond between staff and residents, fostering a sense of stability and care.

LD CARE

Person Centred Bespoke Support Plans

At LD Care, we develop tailored support plans through a thorough needs assessment and transition process that involves input from family, previous carers, professionals, and our own assessments. These plans are designed to meet each individual’s specific needs and are continuously monitored and reviewed by our staff, key workers, community learning disability specialists, friends, and family members.

The support plans are regularly updated as needs evolve, with input from a multidisciplinary team, including specialist LD nurses, psychiatrists, occupational therapists, physiotherapists, dietitians, speech and language therapists, GPs, epilepsy specialists, and behaviour analysts.

Our focus is to ensure individuals are safe and protected while also promoting independence. By supporting residents in making choices and decisions, we empower them to actively participate in their daily lives and enhance their quality of life.

Behaviour Management Programs

At LD Care, we create bespoke behavioural management programs tailored to each individual’s specific needs. We thoroughly assess and analyse behaviours and their possible triggers, developing proactive approaches to reduce their occurrence and foster alternative methods of communication.

Our approach emphasizes proactive positive behaviour support and intensive interaction, rather than just relying solely on reactive strategies to manage behaviour. Through these personalized plans, we’ve seen significant reductions in the frequency and intensity of challenging behaviours, while helping individuals build confidence, independence, and an improved quality of life.

Independent Living, Privacy & Dignity

At LD Care, we prioritize fostering independence by encouraging residents to do as much for themselves as possible. Moving away from an institutional model, we promote dignity and a higher quality of life by empowering individuals, no matter their needs, to take an active role in their daily routines.

Our premises are thoughtfully designed to cater to the safety and well-being of our residents. From the room layouts to the sensory-friendly colour schemes, every aspect of the environment is tailored to create a calming and supportive space, promoting both comfort and security.

Risk Assessment and Management

At LD Care, our ethos is centred on empowering residents to lead fulfilling, meaningful lives by exercising their rights, making choices, and fostering independence. While we encourage fun, exciting, and purposeful experiences, we also ensure that residents are safe and protected through thorough risk assessments and effective risk minimization strategies.

Professional Multidisciplinary and Family Involvement

Professional and family involvement is a cornerstone of LD Care. From the initial assessment process, we engage all professionals and families to gain a full understanding of each resident’s needs. We encourage regular family visits, facilitate home visits, and involve families in special occasions. Their input is valued in shaping care plans and day-to-day support.
